Good Faith Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Session Information
My hours are Monday through Thursday 10am-7pm. Please call or email to inquire about my fee. I don’t take insurance but, as an out of network provider, I am able to offer you billing information that you are able to submit to your insurance provider for reimbursement at their out-of-network rate (each plan is unique). Generally this works out to be around 30-50% depending on the plan. I take Visa, Mastercard, American Express, Discover, cash, and check as payment for sessions. I do maintain a limited number of sliding scale fee spots on my schedule so please feel free to inquire if those are currently available based on need.
My office location is in the AGC building at 1200 Westlake ave. N. #407, Seattle, WA, 98109. There is ample paid parking in front of my building and a Starbucks in the lobby for coffee and snacks when desired. You can reach me at 360-529-0862 with any questions or to set up an appointment. Before our session I will send you a link to my online portal. This link, once you sign in, will give you access to the forms required for us to get started, forms so that I can get to know you and your situation better, and to help us set the groundwork for positive and productive work together. I use a HIPAA compliant electronic healthcare record system that allows me to provide forms, receipts, and statements to make streamline our work together and to make it easier for you to seek reimbursement when desired.
